Participant-Directed Homemaker Personal Care (HPC) at Sustainers Healthcare empowers individuals to take an active role in their care by giving them greater choice and control over how services are delivered. This model allows participants to help select their caregivers, set schedules, and direct daily care activities in alignment with their approved care plan. Through participant direction, individuals receive assistance with daily living tasks such as personal hygiene, grooming, mobility support, meal preparation, and household activities—while maintaining independence and personal autonomy. Our team provides guidance, oversight, and administrative support to ensure services remain compliant with waiver guidelines and healthcare standards. At Sustainers Healthcare, we believe care should be shaped by the individual. Participant-Directed HPC promotes dignity, flexibility, and self-determination, enabling individuals to receive support in a way that best fits their lifestyle while ensuring safety, quality, and continuity of care.
